New patients are welcomed in our practice. Our practice continues to grow by referrals from our patients. We appreciate the trust you have placed in us. We will make every attempt to honor that trust by providing the quality of dental care you require and deserve. One of our primary concerns will be to make you feel comfortable in our office. We will make every effort to insure successful treatment.
Total dental health is dependent on sound teeth and healthy supporting bone. Additionally, occlusion (the way teeth fit together) has a great deal to do with the life expectancy of the teeth. Our goal is to eliminate, as much as humanly possible, problems in all these areas.
But we need your help. If you will work with us, you will have a healthier mouth now and in the future. Without your help, we cannot promise that you will remain free of dental disease. In fact, without your cooperation, we can do very little.
We hope you share in the belief that regular preventive dental health care is a sound investment. While the responsibility for returning for regular treatment rests primarily with you, we will send a postcard when it is time for you to return. If you have not already done so, please telephone for an appointment when you receive this notice. At the time of your recall visit, please advise the receptionist of any change in the following areas: (1) address, (2) telephone, (3) health, (4) medication being taken, (5) marital status, (6) employment, or (7) insurance coverage.
All patients are encouraged to call with any questions they have concerning dental problems. Our office staff is well qualified to answer most questions. A call requiring direct consultation with Dr. Wagner will be returned as soon as time permits.
NO SMOKING POLICY
Exposure to tobacco smoke is detrimental to you and those around you. Please refrain from smoking in our office.
