The fee for treatment is due and payable at the conclusion of each appointment. Services may be charged to your Visa, MasterCard, Discover, or American Express credit card.
INSURANCE
As a convenience to you, our office will submit charges to your insurance carrier, but we consider the patient primarily responsible for the account. In other words, the service provided by the dentist amounts to an agreement between the patient and the office. The insurance relationship constitutes an agreement between the carrier and the patient. Due to the wide variety of insurance policies, we cannot know the details of your individual policy. The portion of the fee not covered by insurance, the co-payment, and the deductible are to be paid by the patient at the time of service. If necessary, pre-treatment authorization will be submitted to your carrier to establish eligibility and estimates of their coverage.
